Advanced Directives

The following resources may be helpful to you.

An advance directive is a written document, such as a living will, instructing the center regarding what kind of care you desire in the event that certain circumstances arise and you are unable to speak or communicate your desires.

Each state has its own form for advance directives, giving you questions to answer and specific things that you can choose to accept or reject, but you can always add additional information about your wishes if the form does not include everything you’re concerned about.

There are organizations that offer information about advance directives and others that offer forms for free or for purchase. The following resources may be helpful to you. EmpRes Healthcare makes no representation regarding the quality of these organizations and websites or any advance directive forms that may be available through these organizations or websites.
